Employees’ State Insurance Corporation, Regional Office Bihar, invites expressions of interest from government, PSU, PM-JAY/CGHS-empanelled hospitals and private healthcare organizations for empanelment to provide cashless super-speciality and secondary care services in Masaurhi, Patna district. Services are paid at the applicable CGHS, ESIC, AIIMS or hospital rates, whichever is lower. The initial empanelment period is two years, extendable by one year by mutual consent.

- Documents required: Application form as per Annexure A1 or A3, as applicable, Undertaking as per Annexure B, Documents listed in Annexure C checklist, Online EMD transaction report including UTR number, Valid NABH/NABL accreditation certificate and scope of services, where applicable, Valid blood bank licence or outsourced agreement, where applicable, Audited balance sheets and profit and loss accounts for financial years 2023-24 and 2024-25, or Chartered Accountant certificate for annual turnover where applicable, Valid State registration certificate or registration with local bodies, Fire Clearance Certificate issued by government authorities, Evidence of compliance with Bio-Medical Waste management rules and State Pollution Control Board rules, Valid registration for Organ and Tissue Transplant Facilities, where applicable, Valid registration under PC PNDT Act, where applicable, Valid AERB/BARC approval for tie-up for radiological investigations/radiotherapy, where applicable, Complete hospital rate list, duly signed and stamped, Due authorization for an authorized signatory, where applicable
