Description
Hiring of one all-inclusive Type D Advanced Life Support ambulance service for Divisional Railway Hospital, Guntakal, for 24 months. The ambulance must be available 24 hours a day, seven days a week, including holidays, and provide a single-stretcher premium van with the required medical equipment and staffing.

- Documents required: Vehicle registration details document; vehicle must be registered in the tenderer's name, or a lease agreement with the vehicle owner/cab aggregator for the contract tenure, Document showing that the vehicle is registered as a commercial vehicle, GST registration certificate (REG-06) or self-declaration of latest turnover attested by a Chartered Accountant, Undertaking that the bidder is not under liquidation, court receivership or similar proceedings and is not bankrupt, Undertaking confirming valid fitness certificate, commercial license with permit covering all States in SCOR Railway jurisdiction, valid insurance, pollution-standard compliance and other applicable statutory obligations, Supporting documents to prove eligibility for exemption from experience or turnover criteria, if exemption is sought, List of ongoing vehicle hire contracts with Railways, government departments and PSUs in the same city/location/agglomeration, including required contract and vehicle details; submission is mandatory even if nil, Police verification certificate for the driver, to be submitted within 15 days of employment
