Description
Expression of interest to empanel hospitals and healthcare organizations to provide super-speciality and secondary care services on a cashless basis to ESIC beneficiaries in Madhubani district, Bihar. Services are paid at CGHS, ESIC, AIIMS or hospital rates, whichever is lower. Empanelment is initially for two years, extendable by one year with mutual consent.

- Certifications: Valid NABH/NABL accreditation (for X and Y cities; preferentially for Z cities), Valid State registration or local body registration, Fire Clearance Certificate, Bio-Medical Waste management compliance, Valid blood bank licence or outsourced agreement, where applicable, PC PNDT registration, where applicable, AERB/BARC approval, where applicable
- Documents required: Application form (Annexure A1 or Annexure A3, as applicable), Undertaking (Annexure B), Documents listed in Annexure C, NABH/NABL accreditation certificate and scope of services, Valid blood bank licence or outsourced agreement, where applicable, Audited balance sheets and profit and loss accounts for financial years 2023-24 and 2024-25, or chartered accountant certificate for annual turnover, Valid State registration certificate or registration with local bodies, Fire Clearance Certificate, Evidence of compliance with Bio-Medical Waste management and State Pollution Control Board rules, Valid registration under PC PNDT Act, where applicable, Valid AERB/BARC approval for radiological investigations/radiotherapy tie-up, where applicable, Complete hospital rate list, signed and stamped, Online EMD transaction report including UTR number, Authorization document for an authorized signatory, where applicable
