Description
ESIC Regional Office Bihar invites eligible government, PSU, PM-JAY/CGHS-empanelled hospitals and private healthcare organizations to apply for empanelment to provide cashless super-speciality and secondary-care services to ESIC beneficiaries in West Champaran district. Services are paid at the applicable CGHS, ESIC, AIIMS or hospital rates, whichever is lower. Empanelment is initially for two years, extendable by one year by mutual consent.

- Certifications: NABH accreditation, NABL accreditation, Valid blood bank licence, where applicable, Fire Clearance Certificate, Bio-Medical Waste management compliance, State registration certificate, PC PNDT registration, where applicable, AERB/BARC approval, where applicable
- Documents required: Application form (Annexure A1 or A3, as applicable), Undertaking (Annexure B), Documents listed in Annexure C, NABH/NABL accreditation certificate and scope of services, Valid blood bank licence or outsourced agreement, where applicable, Audited balance sheet and profit and loss account for financial years 2023-24 and 2024-25, or Chartered Accountant certificate for annual turnover where applicable, Valid State registration certificate or registration with local bodies, Fire Clearance Certificate, Evidence of compliance with Bio-Medical Waste management and State Pollution Control Board rules, Organ and Tissue Transplant Facilities registration certificate, where applicable, PC PNDT registration, where applicable, AERB/BARC approval for radiological investigations/radiotherapy tie-up, where applicable, Complete rate list, duly signed and stamped, Online EMD transaction report including UTR number, Authorization document for authorized signatory, where applicable
