ESIC Regional Office Bihar invites expressions of interest from government, PSU, PM-JAY/CGHS-empanelled and private healthcare organizations for empanelment to provide cashless super-speciality and secondary-care services to ESIC beneficiaries in Munger district. Services are paid at the applicable CGHS, ESIC, AIIMS or hospital rates, whichever is lower.

- Certifications: NABH/NABL accreditation, as applicable, Fire Clearance Certificate, Bio-Medical Waste management compliance, State registration or local-body registration
- Documents required: Application form (Annexure A1 or A3, as applicable), Undertaking (Annexure B), Documents listed in Annexure C, NABH/NABL accreditation certificate and scope of services, where applicable, Valid blood bank licence or outsourced agreement, where applicable, Audited balance sheets and profit and loss accounts for financial years 2023-24 and 2024-25, or chartered accountant certificate for annual turnover, Valid State registration certificate or registration with local bodies, Fire Clearance Certificate, Evidence of compliance with Bio-Medical Waste Management Rules and State Pollution Control Board rules, Valid registration certificate for Organ and Tissue Transplant Facilities, where applicable, Valid registration under PC PNDT Act, where applicable, Valid AERB/BARC approval for radiological investigations or radiotherapy tie-up, where applicable, Online EMD transaction report including UTR number, Complete hospital rate list, signed and stamped, Authorization document for an authorized signatory, where applicable
