Description
ESIC Regional Office Bihar invites hospitals and healthcare organizations to apply for empanelment to provide super speciality, secondary care, diagnostic and related services on a cashless basis to ESIC beneficiaries in Sheohar district. Services are paid at the applicable CGHS, ESIC, AIIMS or hospital rates, whichever is lower. The empanelment is initially for two years and may be extended by one year by mutual consent.

- Documents required: Application form (Annexure A1 or A3, as applicable), Undertaking (Annexure B), Documents listed in Annexure C, NABH/NABL accreditation certificate and scope of services, where applicable, Valid blood bank licence or outsourced agreement, where applicable, Audited balance sheets and profit and loss accounts for financial years 2023-24 and 2024-25, or Chartered Accountant certificate for annual turnover, Valid State registration certificate or registration with local bodies, Fire Clearance Certificate, Evidence of compliance with Bio-Medical Waste Management Rules and State Pollution Control Board rules, Valid registration under the PC PNDT Act, where applicable, Valid AERB/BARC approval for radiological investigations or radiotherapy tie-up, where applicable, Complete rate list, signed and stamped, Online EMD transaction report including UTR number, Authorization document for an authorized signatory, where applicable
