Description
ESIC invites eligible government, PSU, PM-JAY/CGHS-empanelled hospitals and private healthcare organizations to apply for empanelment to provide super-speciality and secondary-care services on a cashless basis to ESIC beneficiaries in Sheikhpura district, Bihar. Services are paid at the applicable CGHS, ESIC, AIIMS or hospital rates, whichever is lower. Empanelment is initially for two years, extendable by one year with mutual consent.

- Certifications: NABH/NABL accreditation, as applicable, Valid State registration or local-body registration, Fire Clearance Certificate, Bio-Medical Waste management and State Pollution Control Board compliance, PC PNDT registration, if applicable, AERB/BARC approval, if applicable, Organ and Tissue Transplant Facilities registration, wherever applicable
- Documents required: Application form (Annexure A1 or Annexure A3, as applicable), Undertaking (Annexure B), Checklist documents (Annexure C), Online EMD transaction report including UTR number, Valid NABH/NABL accreditation certificate and scope of services, as applicable, Valid blood bank licence or outsourced agreement, as applicable, Audited balance sheet and profit and loss account for financial years 2023-24 and 2024-25, or Chartered Accountant certificate for annual turnover if the previous year's audited statement is unavailable, Valid State registration certificate or registration with local bodies, Fire Clearance Certificate issued by government authorities, Evidence of compliance with Bio-Medical Waste management rules and State Pollution Control Board rules, Valid registration under the PC PNDT Act, if applicable, Valid AERB/BARC approval for tie-up for radiological investigations/radiotherapy, if applicable, Valid certificate of registration for Organ and Tissue Transplant Facilities, wherever applicable, Complete hospital rate list, duly signed and stamped, Due authorization for an authorized signatory, if applicable
