Description
Supply, installation, testing and commissioning of 10 transport ventilators, including operator training, statutory clearances, 3-year warranty and 2-year comprehensive maintenance after warranty. Delivery is required within 60 days at G.B. Pant Hospital, Sri Vijaya Puram, South Andaman.

- Documents required: Experience Criteria, Past Performance, Bidder Turnover, Certificate (Requested in ATC), OEM Authorization Certificate, OEM Annual Turnover, Additional Doc 1, Audited balance sheets or Chartered Accountant/Cost Accountant turnover certificate, Relevant contracts and delivery acceptance certificates (CRAC) supporting experience and past performance, Bidder undertaking confirming it is not under liquidation, court receivership, bankrupt or similar proceedings, Product data sheet, OEM authorization form/certificate, where applicable, Valid CDSCO or concerned State/UT Licensing Authority licence for the offered device, Valid ISO 13485 certificate for the relevant manufacturing site, BIS licence, type test certificate, approval certificates and other applicable product certificates, IEC 60601-1 or corresponding Indian Standard test reports or conformity documents, Medical Device Rules (MDR) 2017 compliance documents, medical device licence and batch/product test reports, Undertaking for 2 years of comprehensive maintenance after warranty, Service centre details near consignee destinations, Undertaking for post-warranty comprehensive maintenance at no more than 10% of contract price per annum
