Description
Supply of 15,000 Three Way Stopcocks across four schedules for the Directorate of Health Services, Andaman and Nicobar Administration. Schedule quantities are 6,000, 5,000, 2,000 and 2,000. Delivery is required within 26 days for the first schedule, 30 days for the remaining schedules. Bid validity is 30 days; past performance requirement is 60% of bid quantity. Reverse auction is enabled.

- Documents required: Copies of relevant contracts and delivery acceptance certificates like CRAC, Copies of relevant contracts proving supply of cumulative order quantity in any one financial year, Supporting documents to prove eligibility for exemption from Experience / Turnover Criteria, Valid drug license from competent authority for manufacture/import to sale, For resellers: valid drug license for sale or distribution and valid manufacturer/import drug license, GST Registration, PAN Card, Relevant certificates including BIS licence, type test certificate, approval certificates and other certificates prescribed in the Product Specification, OEM certificate stating percentage of local content and locations of local value addition
