Supply of four schedules of MGIT diagnostic kits and culture media, total quantity 44 packets, for delivery to Dehradun within 30 days. Item-wise evaluation applies. EMD is required for each schedule, and the successful bidder must furnish a 5% ePBG for 5 months.

- Documents required: Experience Criteria, Past Performance, Bidder Turnover, Certificate (Requested in ATC), OEM Authorization Certificate, OEM Annual Turnover, Compliance of BoQ, Supporting documents for exemption from experience or turnover criteria, if applicable, Relevant contracts and delivery acceptance certificates such as CRAC, Compliance sheet, Valid Drug License for the quoted drug by CDSCO/DCGI, Duly notarized undertaking certificate as per attached, Ceiling price fixed by Central/State Government, OEM authorization letter with office seal, signatory name, designation and phone number, Batch test certificate from a Government-authorized laboratory/NABL
