- Documents required: Proof of supply of similar LOCA qualified Wide Range Area Radiation Monitor (100 mR/hr-100000 R/hr) to an NPCIL unit: purchase order copy and one proof of supply such as delivery challan, LR, invoice copy, performance report, PBG copy, or FTR copy, Approved certificate for standard radiation calibration facility at bidder's own premises, Duly filled and signed technical compliance sheet for the Buyer Specification Document and Buyer Uploaded ATC Document, Data sheet/details of offered product with declaration that the offered product/make/model meets the bid technical specification, Technical specification document and qualification criteria, duly signed and stamped, Drawing/sketch of offered product, Catalogue of quoted product, OEM certificate stating percentage of local content and locations of local value addition, Declaration as per Annexure-II for one bid per bidder, Seller declaration that the offered product is manufactured by the bidder, for MSE purchase preference, PAN card copy, GSTIN copy, Cancelled cheque copy, Bank-certified EFT mandate, QAP within 15 days from issue of purchase order
