Quantities may vary according to hospital requirements. The selected bidder must refill cylinders within 24 hours of being notified and provide delivery to the hospital oxygen control room/store.

- Documents required: Shop and Establishment Act registration certificate, GST registration certificate, PAN registration certificate, Valid drug licence effective for at least the next 1 year, Income tax returns for financial years 2023-24, 2024-25 and 2025-26, Evidence of at least 3 years of medical oxygen supply experience at a hospital with at least 100 beds, Valid cylinder storage licence issued by the Chief Controller of Explosives, Government of India, Notarized affidavit on Rs. 100 stamp paper confirming the bidder is not blacklisted by any institution, Chartered Accountant certificate confirming annual turnover of Rs. 50 lakh in financial years 2022-23, 2023-24 and 2024-25, Copies of testing and impurity records for the previous 5 years, Affidavit on Rs. 100 stamp paper in the prescribed supply declaration format, Certificate stating daily medical oxygen production in tonnes, cylinder repair capacity, and quality control and gas analysis arrangements as per IP standards, Bank account details for direct payment
