Description
Supply, installation, testing and commissioning of 310 LED street lights, including training of operators and statutory clearances if required, at different places in Ahraura, Mirzapur. Delivery period is 30 days.

- Documents required: Certified audited balance sheets or Chartered Accountant/Cost Accountant turnover certificate, Contracts/work orders and delivery acceptance certificates or CRAC supporting experience, Contracts supporting supply of 50% of bid quantity in at least one of the last three financial years, Certificate requested in ATC, OEM Authorization Certificate, OEM annual turnover evidence, Additional Doc 1, Valid supporting documents for any claimed EMD exemption, Undertaking that bidder is not under liquidation, court receivership, bankruptcy or similar proceedings, Certificate confirming registered office in India for imported products, ISO 9001 certificate of bidder or OEM, BIS licence, type test certificate, approval certificates and other product specification certificates, LM79, LM80 and CAT:AAA category certificate from OEM, Dedicated/toll-free service support telephone number and service support escalation matrix, Acceptance letter for all bid conditions on authorized firm letterhead, Valid GSTIN registration certificate, Blacklisting undertaking on Rs. 100 stamp paper and self-declaration on Rs. 10 revenue stamp, duly notarized, Work order copy, invoice copy attested by government department or GeM invoice, and customer receipt acceptance certificate, At least three satisfactory performance certificates from the respective buyer organization for the same or similar category in the last three years, Last three years' ITR acknowledgements, PAN, Aadhaar and registration certificate, self-attested, Hard copy of EMD in the form of FDR in favour of Executive Officer, Nagar Palika Parishad Ahraura
