Supply, installation, testing and commissioning of 10 ICU ventilators, including operator training and statutory clearances where required. Delivery is required within 60 days to G.B. Pant Hospital, Sri Vijaya Puram, South Andaman. The equipment requires a 5-year warranty and 5-year comprehensive maintenance after warranty.

- Documents required: Experience Criteria evidence, including relevant contracts and delivery acceptance certificates, Past Performance evidence, including relevant contracts, Certified audited balance sheets or Chartered Accountant/Cost Accountant turnover certificate for bidder, OEM annual turnover evidence, Certificate requested in ATC, OEM Authorization Certificate, Additional Doc 1 requested in ATC, Additional Doc 2 requested in ATC, Product data sheet, Undertaking that bidder is not under liquidation, court receivership, bankruptcy or similar proceedings, Valid applicable CDSCO or State/UT Licensing Authority licence under Medical Devices Rules, 2017, Valid ISO 13485 certificate covering the relevant manufacturing site and medical-device activities, IEC 60601-1 or corresponding Indian Standard test reports or conformity documents, BIS licence, type test certificate, approval certificates and other certificates prescribed in the product specification, Undertaking for 5 years of warranty and 5 years of CAMC after warranty
