Description
Supply of 8,000 packets of glucometer strips to the Directorate of Health Services, Andaman and Nicobar Administration. Each pack contains 50 strips. One compatible glucometer must be provided free of cost for every 10 packs (500 strips). Delivery is scheduled in four lots of 2,000 packets.

- Certifications: Compliance to Drugs and Cosmetic Act 1940 and Medical Device Rules (MDR) 2017 as amended till date, Valid Medical Device license for the product issued from the competent authority
- Documents required: Experience Criteria, Bidder Turnover, Certificate (Requested in ATC), OEM Authorization Certificate, OEM Annual Turnover, Supporting documents to prove eligibility for exemption from Experience / Turnover Criteria, if exemption is sought, Relevant contracts and delivery acceptance certificates such as CRAC, Certified audited balance sheets or a Chartered Accountant / Cost Accountant certificate indicating turnover, OEM certificate stating percentage of local content and locations of local value addition, if claiming MII purchase preference, Valid drug license for manufacturing / import of the product, Valid drug license for sale or distribution, in case of reseller, PAN Card, GST Registration Certificate, Relevant certificates including BIS licence, type test certificate, approval certificates and other certificates prescribed in the product specification, Valid Medical Device license for the product
