Description
Indira Gandhi Medical College and Hospital, Shimla invites manufacturers or their authorized stockists/distributors to empanelment for supplying orthopedic artificial appliances and implants on a patient-to-patient basis under government schemes. Supplies are required within 30 minutes of telephonic orders. The initial rate contract is for two years, extendable by up to one year at a time based on satisfactory performance.

- Documents required: Company/firm registration documents, GST registration, PAN, ISO certification, Manufacturing license (manufacturer) or tender-specific sole authorization (stockist/dealer), Completed Form-A, Chartered Accountant-certified average annual turnover certificate for 2023-24 to 2025-26, Affidavit regarding blacklisting and product status, Completed Annexure-IV with details of quoted products, Undertaking accepting tender terms and conditions, Manufacturer authorization letter, Rate reasonability certificate/undertaking, Proof of tender document fee payment, Proof of EMD deposit, Power of Attorney, board resolution, or other authorization for the signatory
