- Documents required: Tender Form as per Annexure-1, Drug license issued by FDA and renewed supplier shop establishment registration certificate, Manufacturer/authorized dealer authority letter, Performance statement for 2022-23, 2023-24 and 2024-25 supported by purchase orders, Income Tax Returns for the last 3 years, Annual turnover statement certified by a Chartered Accountant, Balance sheets and Profit and Loss accounts for 2022-23, 2023-24 and 2024-25 certified by the Auditor, GST Registration Certificate and latest GST paid receipt, PAN Card, Experience Certificate of ₹7 crore per year for the past three financial years, Affidavit stating quoted rates are not higher than DPCO, NPPA or MRP, Affidavit stating the firm has not been found guilty of malpractice, misconduct, or blacklisted/debarred, Photocopy of online tender fee, Photocopy of online EMD, Security Deposit Form, Banker's Certificate (Form A) from a scheduled nationalized bank, Valid WHO and DCGI Certificate of manufacturer, NABL Test report, Attested drug manufacturing license for each quoted product, Valid WHO and DCGI Certificate with product list / COPP, Original exclusive authorization letter from manufacturer, if bidder is a distributor, Latest market standing of quoted product for 2022-23, 2023-24 and 2024-25, certified by FDA, Valid Non-conviction Certificate issued by the Drugs Commissioner of the State, Applicable WHO GMP/CE/ISO/BIS certificates and other applicable accreditations, Samples of medicines and surgical items with sample approval letter as required, Original documents and required affidavits
