- Documents required: Bidder information form (Annexure I), Earnest Money Deposit and tender fee payment proof, or eligible Maharashtra MSME manufacturer exemption documents, Original product catalogues/brochures and technical data sheets, GST registration certificate and GST clearance certificate/GST3BR, Audited financial statements and Chartered Accountant-certified turnover and net-worth documents for the last three financial years, Purchase order copies supporting completion/delivery of at least three similar works for government, semi-government, PSU, local self-government or autonomous bodies in India during the last five years, Manufacturer authorization letter (Annexure II), where applicable, Valid BIS/ISO licences under the Medical Devices Rules, 2017 (MD-15 for imported or MD-5/MD-9 for indigenous devices, and MD-42), ISO 13485:2016 certificate for the quoted device's manufacturing site, US FDA / CE certificates where required by the technical specifications, User list of quoted model (Annexure IV) and purchase orders for the quoted model from the last three financial years, Make and model details for all equipment in the scope of supply, Details of at least two Maharashtra service centres (Annexure V), Terms and conditions acceptance certificate (Annexure VI), Site survey report signed by the appropriate authority and preliminary drawings, Signed and stamped Common Set of Deviations (CSD), if issued, Affidavit on non-judicial stamp paper of Rs. 500 regarding prevailing market price, Affidavit on non-judicial stamp paper of Rs. 100 regarding no malpractice, misconduct, blacklisting or debarment
