Appointment of supplier for food grains, grocery, vegetables, fruits, dairy, and other eatable items to Regional Mental Hospitals in Pune, Thane, Nagpur, and Ratnagiri for three years (2026-27 to 2028-29).

- Documents required: Tender Form as per Annexure-I, Power of attorney, board resolution, or other authorization for the officer and responsible person representing the tenderer, Attested supplier's licence issued by the respective State Government for each quoted product, renewed and with quoted items highlighted, Certificate of Supply Business Registration, Registration with FDA under the Food Safety and Standards Act, 2006, GST Registration certificate, paid GST challans for financial years 2023-24, 2024-25 and 2025-26, and latest GSTR-3B filed, Income Tax registration, PAN, and income tax returns for financial years 2023-24, 2024-25 and 2025-26, Annual turnover statement for financial years 2023-24, 2024-25 and 2025-26, certified by a Chartered Accountant with UDIN, Audited Balance Sheets and Profit and Loss Accounts for financial years 2023-24, 2024-25 and 2025-26, certified by an auditor with UDIN, Copies of contracts or work orders and documentary evidence of successful execution or completion of past supply experience, with client names, addresses and contact details, Past performance for the last three years in Annexure-II format, supported by purchase orders or satisfactory certificates from clients, Notarised bank-statement extract showing payments received from the same clients (may be submitted), Affidavit on Rs. 500 non-judicial stamp paper confirming the firm has not been found guilty of malpractice, misconduct, or blacklisted/debarred in the past three years, Annexure B, Attested valid registration of the manufacturing firm for the offered product under D.G.S. & D., S.S.I. or N.S.I.C., if applicable, including EM Part II certificate, Government Resolution and registration certificates supporting any claimed Maharashtra Government preference, if applicable, Information and evidential documents on ownership and existence of processing and manufacturing machinery for quality assurance, as per Annexure-IV, Other relevant documents required under the tender terms and conditions
