Supply, installation, testing and commissioning of one Bipolar Electrohydraulic Lithotripter with probes for AIIMS Kalyani, including operator training and statutory clearances where required. Delivery is required within 60 days. The scope includes 15 Biliary Electrohydraulic Lithotripter Probes. The equipment has a 2-year warranty followed by 8 years of comprehensive maintenance.

- Documents required: Experience Criteria, Past Performance, Bidder Turnover, Certificate (Requested in ATC), OEM Authorization Certificate, OEM Annual Turnover, Additional Doc 1 (Requested in ATC), Compliance of BoQ specification and supporting document from seller, Supporting documents to prove eligibility for exemption from Experience / Turnover Criteria, if applicable, Relevant contracts and delivery acceptance certificates such as CRAC, Certified audited balance sheets or Chartered Accountant / Cost Accountant turnover certificate, Make in India specific authorisation certificate, Undertaking that bidder is not under liquidation, court receivership, bankruptcy or similar proceedings, Manufacturer Authorization Form / Certificate, where applicable, CDSCO or State Licensing Authority Manufacturing License / Import License, OEM certificate confirming registered office in India for imported products, OEM certificate stating local content percentage and locations of local value addition, Undertaking that consumable prices remain fixed during warranty and CMC periods and consumables are genuine OEM-compatible, Bidder undertaking for land-border procurement restrictions, where applicable
