Supply of 60 dry-stored bioprosthetic bovine pericardial mitral valves in assorted sizes 25-33mm to Army Hospital R&R, Delhi Cantt. Delivery is required within 30 days. EMD is not required; ePBG of 3% for 18 months is required.

- Documents required: Experience Criteria, Past Performance, Bidder Turnover, Certificate (Requested in ATC), OEM Authorization Certificate, OEM Annual Turnover, Compliance of BoQ, Supporting documents for exemption from Experience / Turnover Criteria, if applicable, Certified audited balance sheets or Chartered Accountant / Cost Accountant turnover certificate, Relevant contracts and delivery acceptance certificates, Undertaking that bidder is not under liquidation, court receivership, bankrupt or similar proceedings, PAN Card, GSTIN, Cancelled Cheque, Bank-certified EFT Mandate, OEM authorization form/certificate, Registration/empanelment approval with Army Hospital and any other Service Hospital, Certificate confirming registered office in India for imported products, ISO 9001 certification, Material Test Certificate, BIS licence, type test certificate, approval certificates and other product specification certificates, Positive OEM net worth evidence, Local content undertaking and certificate, where applicable
