Description
Supply, installation, testing and commissioning of one Bioimpedance Spectroscopy (BIS) system, including operator training and statutory clearances if required. Includes a 2-year warranty and 8 years of post-warranty AMC.

- Documents required: Experience Criteria documents, including relevant contracts and delivery acceptance certificates, Past Performance documents, including relevant contracts proving supply quantity, Bidder turnover evidence: certified audited balance sheets or Chartered Accountant/Cost Accountant certificate, Certificate requested in ATC, OEM Authorization Certificate, Compliance of BoQ specification and supporting documents, Supporting documents for exemption from experience or turnover criteria, if claimed, Tender Document Checklist, Technical Compliance Sheet, Completed technical specifications and additional parameters with relevant data in the remarks column, EMD receipt, EMD demand draft, or MSE/NSIC/Startup registration certificate, Income Tax Return acknowledgements for the last 3 years, Profit and Loss account and Balance Sheet for the last 3 years, PAN card, GST Registration Certificate, Technical catalogue, brochure, literature and specification sheet, with product image or drawing showing dimensions, Bid-specific valid OEM Manufacturer Authorization, where applicable, Make in India certificate stating local content percentage and value-addition locations, Notarized affidavit on non-judicial stamp paper of Rs. 100 and on letterhead, Disclosure of existing customers in India, Signed and stamped GeM Tender Document, Declaration of country of origin and local content bifurcation, Compliance with O.M. F.7/10/2021-PPD dated 23/02/2023 in the prescribed format, Functional service centre details for the consignee's state, where applicable, Product data sheet, Undertaking that bidder is not under liquidation, court receivership or bankruptcy, Relevant certificates including BIS licence, type test certificate and approval certificates, as prescribed, Proof of past experience/project execution, such as contract and invoices, client execution certificate, or other execution evidence, Signed Integrity Pact, GST invoice and GST portal payment confirmation at invoice generation
