Supply of 12 packets of biochemistry reagent kit for human samples, for Hemoglobin A1C testing. Delivery is required within 15 days to the Regional Medical Research Centre in Sri Vijaya Puram. The kit must meet the stated sample, packaging, certification and catalogue-number requirements.

- Documents required: Experience Criteria, Past Performance, Bidder Turnover, Certificate (Requested in ATC), OEM Authorization Certificate, OEM Annual Turnover, Supporting documents to prove eligibility for exemption from Experience / Turnover Criteria, if exemption is sought, Relevant contracts and delivery acceptance certificates such as CRAC, Certified audited balance sheets or a Chartered Accountant / Cost Accountant certificate indicating OEM turnover, Valid Udyam Certificate and supporting documentary evidence for MSE purchase preference, Experience Certificate for supply to a Government / PSU / renowned private organisation along with Supply / Purchase Order, Make in India specific authorisation certificate, EMD exemption certificate, if registered under MSME or NSIC
