Supply, installation, testing and commissioning of one advanced phacoemulsification machine for cataract surgery, with 2-year comprehensive warranty and 5-year comprehensive maintenance after warranty. Delivery is required within 45 days. Pre-dispatch inspection by RITES is required.

- Documents required: Experience Criteria, Past Performance, Certificate (Requested in ATC), OEM Authorization Certificate, Relevant contracts and delivery acceptance certificates such as CRAC, EMD exemption supporting documents, if exemption is claimed, Compliance to detailed technical specifications document attached with the bid, OEM authorization, Proof of supply for past performance, Year-wise CAMC rates for 5 years after warranty, Undertaking for product demonstration during technical evaluation, Relevant certificates including BIS licence, type test certificate and approval certificates, Medical Device Licence, OEM certificate of local content and locations of local value addition, if claiming MII preference
